Under Federal law, we are directed to inform you of your right to make advance directives- instructions which ensure your health care decisions are carried out in the event you become unable to communicate your wishes. As a U.P. Home Health or U.P. Hospice patient, we’ll help you understand the details of this law, and we’ll give you detailed information to help you initiate advance directives.
