Adult/ Geriatric Care: Our nurses focus on the unique health needs of the adult and senior population. Whether to monitor a chronic or acute condition, teach medication management, or ease the transition following surgery, our skilled nursing, therapy and aide services help you maintain your independence and act as a cost-effective alternative to extended hospitalization or nursing home placement.

Rehabilitation - Orthopaedic/Neurological Care: Before your hospital discharge, our nurses and therapists who specialize in home care go to the hospital to participate in the "patient case conferencing meetings" which help establish your rehab program. Your rehab program helps you build strength, in-crease mobility, and learn to work within the limits of new physical challenges. Rehab patients include those recovering from a stroke, heart condition, surgery, hip surgery, hip fracture, or other orthopedic conditions, such as arthritis.

Surgical Care: Whether your doctor has recommended inpatient or outpatient surgery, U.P. Home Health & U.P. Hospice ensures a healthy homecoming. We are there to assist you before and after surgery. And as soon as you are home, we’ll be on hand to help with surgical dressings and medications (including I.V. antibiotics).

OB/GYN And Pediatric Care: Specialty nurses provide childbirth education classes and in-home care to support every stage of pregnancy. For mothers and children enrolled in the Medicaid program, our Maternal Support and Infant Support Services programs supplement regular medical care. As your family grows, our Pediatric Team is there, offering nursing care for short-term, acute illnesses as well as chronic conditions.

Enterostomal, Wound Care And Continence Specialists: U.P. Home Health & U.P. Hospice employs full-time ET nurses with experience in stoma care and family-oriented teaching. ET nurses provide expertise in the treatment of wounds and "bed sores".

Hospice Care: Hospice offers a family-centered program for terminally ill patients whose focus has shifted to comfort measures and control, ensuring dignity during the final stages of their illness. Hospice is a philosophy that embraces the heart of home-based health care, and it is supported by both skilled services and the caring attention of our Hospice volunteers.

Pre-Natal Care: For some moms, pregnancy brings special challenges:
gestational diabetes
Pre-existing medical conditions which affect pregnancy
Hypertension
Pre-term labor

Your doctor may order home care visits from a nurse. The nurse will monitor the condition of both you and baby and make regular reports back to your doctor. Prenatal care may also include nutritional counseling, social work services, or aides to offer help with personal care.

Pediatric Services: Home health care can help patients of all ages, including children. For infants, our Skilled Nursing Team offers:

Photo-Therapy, or "billi lights"
Developmental Assessments
Intravenous (IV) Therapy Management
Apnea Monitoring
Family teaching to assist with the special needs of premature infants

We also provide comprehensive Therapy, Physical Therapy, and Speech/Language Therapy, which support a complete rehabilitation program for children recovering from an accident or long-term illness.

Who Provides Service? Your U.P. Home Health maternal-child support team includes Nurses, Home Care Aides, and Social Workers. Each has background in the special needs of pregnancy and parenting.
